The season opens with Eugene Pontecorvo denied permission to retire; trapped between the crew and the FBI, he takes his own life. Junior, increasingly confused by dementia, then shoots Tony at home. While Tony lies in a coma, he dreams he is salesman Kevin Finnerty, as Silvio tries to lead the crew. Tony survives and returns talking about a changed outlook, but his old habits soon return. Vito Spatafore is exposed as gay, flees to New Hampshire, and briefly builds a different life with a local cook before returning to New Jersey.

Phil Leotardo makes Vito’s return a test of control and has him killed. Johnny Sack dies of cancer in prison, leaving New York without a settled leader. Tony and Phil clash over business and authority. At home, A.J. drops out of college and struggles after Blanca ends their relationship. Christopher’s movie Cleaver fuels private resentment between him and Tony. After Christopher crashes a car while high, he asks Tony to call a taxi because he will fail a drug test. Tony realizes Christopher has relapsed and suffocates him.

A.J.’s depression deepens into a suicide attempt in the family pool; Tony rescues him, and his parents seek help. The conflict with New York then escalates: Phil orders attacks that leave Bobby dead and Silvio gravely wounded, forcing Tony’s crew into hiding. His men later negotiate peace with Phil’s family, then locate and kill Phil. Afterward Tony visits Junior, who no longer recognizes him. The season ends at Holsten’s with Tony, Carmela, and A.J. seated; Meadow is still parking and approaching the diner when the scene cuts to black. The episode does not show what happens next.
