Christopher celebrates the premiere of Cleaver, but Carmela sees the film's murderous boss and romantic triangle as an attack on Tony. She confronts Christopher about the resemblance to old rumors concerning Tony and Adriana. Christopher denies the connection and pressures screenwriter J.T. Dolan to take responsibility for the story. When J.T. resists, Christopher strikes him with a trophy. J.T. subsequently tells Tony that he borrowed the plot from Born Yesterday. Tony notices the injury and doubts the explanation. In therapy, he admits that he believes Christopher despises him. At Christopher's daughter's baptism, the two men embrace while their distrust remains unresolved.

Johnny Sack receives treatment for lung cancer in a prison hospital. He hopes for more time, but his condition worsens and he dies with Ginny and their daughters beside him. The struggle over New York's leadership continues as Phil initially supports Gerry Torciano rather than seeking the position himself. Doc Santoro has Gerry murdered while he is dining with Silvio, who survives the shooting. Angry that his adviser was endangered, Tony encourages Little Carmine to compete for control again. Little Carmine declines, explaining that his wife fears the consequences of pursuing more power. Phil later speaks bitterly to Butchie on what would have been Billy's birthday and declares that he will make no more compromises.
