After Tony and Carmela separate, he lives at his mother’s old house and pursues Dr. Melfi again. Several men return from prison, including Tony’s cousin Tony Blundetto and the demanding Feech La Manna. Tony wants Blundetto to earn with the crew, but his cousin first chooses massage therapy; Feech immediately challenges Tony’s order. In New York, boss Carmine’s death leaves Johnny Sack and Little Carmine competing for control, while Phil Leotardo and other released men join the dispute.

Tony gives Blundetto extra room out of guilt about their past: Tony avoided prison after a panic attack, while his cousin served a long sentence. Blundetto tries to build a lawful life, but gambling draws him back toward the mob. He accepts work from his former prison friend Angelo, kills Joey Peeps, then ambushes Phil and kills Phil’s younger brother Billy. Tony’s favoritism puts both families at risk, and Johnny Sack demands that he deliver his cousin.

Adriana’s FBI handler tightens the pressure as Christopher grows suspicious and jealous over a rumor that she and Tony had an affair. Tony and Carmela agree to resume living together without repairing their marriage. After Adriana confesses her cooperation, Silvio takes her into the woods and kills her. Tony tracks Blundetto to their uncle’s farm and kills him before meeting Johnny Sack to negotiate peace; FBI agents arrive and arrest Johnny, and Tony flees. The two deaths leave the FBI threat and New York feud unresolved.
