Baldur’s Gate 3: Rescue Volo and Decide About His Eye Surgery

Get inside while the camp will still talk
Volo’s Act One rescue begins in the Goblin Camp courtyard. Travel west through the Blighted Village and use dialogue to pass the goblin guards if you want to explore without making the whole camp hostile. The courtyard and much of the Shattered Sanctum can be visited before resolving the goblin-leader conflict.
Look for Volo performing on the outdoor stage. Speaking to him interrupts the performance; his captor, Gribbo, takes him back inside. Meeting him earlier in the Emerald Grove is not required for this rescue.
Handle this conversation before experimenting with the courtyard’s poisoned-drink route. Poisoning the booze before speaking to Volo or Gribbo can prevent the usual rescue sequence from starting.
Find Gribbo’s cage room
Enter the large doors into the Shattered Sanctum. From Priestess Gut’s central area, head to the small room to the northeast. Gribbo is standing beside Volo’s cage there, around X:333, Y:-8.
Ask about her captive and work toward having him released. You can bargain, use a suitable dialogue check, or obtain the key through theft or combat. The peaceful options let you finish the rescue without clearing the rest of the dungeon. If you choose to pay, the minimum accepted bribe is 250 gold.
Once you have access, open the cage and speak with Volo. Invite him to your camp. He uses an invisibility potion to escape, so you do not need to escort him through every room or start a battle to clear his path.
Back at camp, speak with him to finish the rescue. His reward includes the Blazer of Benevolence. The conversation about your parasite leads to a separate, optional decision.
Decide who will receive the replacement eye
Volo can examine the parasite and offer to research a treatment. After resting, he proposes an operation. It does not remove the tadpole. Continuing the procedure costs the patient their right eye, which Volo replaces with his magical Ersatz Eye.
That character gains permanent See Invisibility. The effect checks nearby invisible creatures within 9 metres, and they can resist detection with a Dexterity saving throw. It is not guaranteed detection of every invisible enemy across an entire room.

The replacement is permanent, including its appearance: the Magic Mirror cannot recolour that eye afterward. A character who has already paid an eye to Auntie Ethel cannot receive this replacement through Volo’s procedure. Wyll cannot undergo it either. Choose the intended patient before entering the surgical conversation.
There are several chances to stop, but the dialogue option “Hold your breath” commits you to the final stage. Decide whether the passive and cosmetic change suit that character before reaching that point.
Continue the other camp rescues
Freeing Volo does not free the Owlbear Cub, settle the Goblin Camp’s leaders or complete the Grove conflict. Those objectives have their own steps. Use the Act One route to finish the branches you want before advancing to the next region.
