Baldur’s Gate 3: Get Awakened From the Zaith’isk

Do the infirmary before making it hostile

The zaith’isk is in the Crèche Y’llek infirmary. Speak to Ghustil Stornugoss and use the device before killing her: if she dies after the conversation but before the attempt, it becomes inoperable. The machine is destroyed after use, so this is one attempt for the character you put inside.

Take care with Lae’zel’s dialogue. She strongly wants the device and her route has its own checks and story pressure. If your goal is the passive on the player character, resolve that conversation deliberately instead of clicking through it with the first character selected.

Pass all three saves

The standard sequence for the player character is DC 12 Intelligence, DC 15 Wisdom, then DC 18 Constitution; class-specific dialogue can offer alternatives. Passing all three grants Awakened, which changes Illithid powers that normally take an Action into Bonus Actions.

This makes a power easier to fit beside an attack or spell, but it is not a universal upgrade. It does not change a Reaction or a power already using a Bonus Action, and you cannot toggle the passive off. A build that relies on its Bonus Action every turn should decide whether the action economy trade is worth it.

Remove any ability penalties

Each failure applies a lasting -2 penalty: Intelligence first, then Wisdom, then Constitution. Long resting does not clear it. The affected character can remove all of these penalties by consuming one Mind Flayer Parasite Specimen, but that is another story/build decision, so do not rely on it by accident.

Prepare saving-throw support before sitting down. After the device breaks, Ghustil can be persuaded that the parasite is dead with the available DC 15 Deception or Performance response. Otherwise she can lock the room and return with guards. That local confrontation does not automatically turn the entire crèche hostile; fast travel can avoid it. If you only want to avoid the procedure’s risks, using the machine is optional.
