Annie has Frenchie remove her tracking device and brings the Boys the Sage Grove lead. Mother's Milk, Frenchie and Kimiko enter the psychiatric facility while Butcher, Hughie and Annie wait outside. They find patients being experimented on with Compound V and recognize Lamplighter working there. Frenchie's confrontation with him breaks containment, leaving the infiltrators trapped alongside the former Seven member as dangerous patients escape into the building.
An escaped patient's attack badly injures Hughie, forcing Annie and Butcher to work together to get him to hospital. Inside, Frenchie and Lamplighter revisit the deaths of Mallory's grandchildren. Lamplighter says he meant to kill Mallory, while Frenchie explains that he abandoned his surveillance to save a friend from an overdose. Their shared failure does not disappear, but cooperation is the only way out. Lamplighter also confirms that Vought wants Compound V to work reliably on adults.
Lamplighter lies to Stormfront to protect the group, and Frenchie later persuades Mallory to keep him alive as a witness. Maeve obtains footage of the abandoned passenger plane, although showing it to Elena exposes how much she has hidden. Stormfront finally tells Homelander that she was Frederick Vought's wife and the first successful Compound V subject. She offers him leadership of her racist vision for the future, and he accepts her intimacy instead of rejecting what she has revealed.
