With Carter away in Africa, County General’s next generation takes on more responsibility. Abby, Neela and Ray become second-year residents and must supervise interns while learning to manage the emergency department’s relentless demands. Neela distinguishes herself during a field emergency and grows closer to Gallant when he returns from Iraq. Their reunion becomes a marriage: she accepts his sudden proposal, though the commitment is still new when they have to decide what his military service means for their future.

Luka becomes chief of emergency medicine and clashes with Victor Clemente, whose increasingly erratic conduct turns a workplace rivalry into a safety concern. Luka eventually intervenes when Clemente’s behavior puts patients at risk. Abby, meanwhile, faces an unexpected pregnancy and decides to have the baby with Luka. Their relationship moves from uncertainty toward family life, even as the job keeps pulling them into difficult choices. Pratt’s leadership is tested by a false blood test he uses to protect his friend Darnell, an act that leaves him suspended and facing consequences.

Gallant chooses to return to Iraq, believing he has a duty to the soldiers he served with, and Neela is hurt that he leaves after they have married. In the penultimate episode, a truck explosion kills him before he can tell his comrades about his wife. Neela learns the news just after Dubenko offers her a surgical residency; she keeps working until she can face the loss at home. At the season’s end, she attends Gallant’s funeral, while a hostage crisis at County leaves Luka, Sam, Alex and others in danger.
