Tony sinks into depression after Pussy disappears, spending much of his time in bed. He becomes interested in Isabella, a young Italian woman apparently staying next door while the Cusamanos are away. She speaks about Avellino, where his grandfather came from, and Tony imagines her nursing a baby named Antonio. When he later asks Cusamano about her, he learns that no such guest was there. Melfi tells him to stop taking lithium and suggests that Isabella represents the nurturing mother he wishes he had. The meetings and the maternal image are revealed as hallucinations rather than an actual relationship.
Junior's assassination plan proceeds through Mikey and intermediary Donnie Paduana, who hires two gunmen. Christopher unknowingly blocks their first opportunity by pulling up near Tony. When Donnie casually remarks that even Tony's mother wants him dead, Junior orders Mikey to silence him. The next day, the gunmen attack Tony as he gets into his car. One accidentally shoots his partner, and Tony fights off the survivor before crashing while escaping. He tells the hospital and his family that he was the victim of a carjacking. Agent Harris offers him protection through cooperation with the FBI, which Tony refuses. Junior and Livia later visit him at home, where Livia appears not to recognise Meadow. Junior remarks on the convenient timing of her apparent memory problems.
