A few months later, Fiona is waitressing with Veronica while trying to build a life without Jimmy. He returns using his real name, now married to Estefania, whose father is a drug dealer; Jimmy later smuggles Estefania’s former boyfriend Marco into the country. Lip’s relationship with Karen shifts when she becomes pregnant and he believes the baby may be his. Frank’s mother, Peggy, comes home from prison on medical furlough, bringing old abuse and new conflict into the house. When Peggy’s cancer advances, she asks Sheila to help end her life, and Sheila does. Frank reconnects with Monica, who returns to the family but secretly stops taking her medication.
Monica spends the Gallaghers’ hidden savings, and a family dinner ends with her attempting suicide; she survives, is sent to psychiatric care, and later escapes with another patient. That same crisis coincides with Karen giving birth to Hymie, whom Karen identifies as Timmy Wong’s child. Karen refuses to raise him, and when prospective adoptive parents withdraw, Sheila and Jody take him from the hospital. Sheila chooses the baby when Karen demands that she choose between them, so Karen leaves. Lip, who had dropped out of school over the pregnancy, returns to finish; he and Fiona make peace. Jimmy and Fiona reunite, while the family turns Frank out after another drunken collapse.
