Seven years after Michael’s apparent death, T-Bag receives a photograph showing him alive in Ogygia prison in Sana’a, Yemen. Lincoln deciphers the word on the message, digs up Michael’s grave and finds no body, then asks C-Note to help him reach Yemen. Sara, now married to Jacob and raising Michael’s son, initially doubts the photograph. At Ogygia, Michael insists he is Kaniel Outis, a prisoner accused of terrorism, and refuses to acknowledge Lincoln. His secret plan depends on escaping amid Yemen’s conflict, with fellow inmates Whip, Sid and Ja drawn into the attempt.

Michael’s escape plan repeatedly goes wrong as Abu Ramal and other prisoners threaten the group. Lincoln, Sheba and C-Note work outside the prison to move them toward safety, while Sara investigates Michael’s return and learns that a powerful figure called Poseidon has been controlling events. Michael explains that after Scylla was handed to the United Nations, Poseidon forced him into covert work and used Sara’s imprisonment to make him comply; Michael faked his death during her escape and was later sent to Ogygia under a false identity. Sid dies helping the group evade their pursuers, and the escapees leave Yemen by separate routes as the crisis closes in.

Back in the United States, Michael and Sara reunite, but the danger follows them home. They discover Jacob is Poseidon, who has been manipulating Michael and threatening Sara and their son. T-Bag has learned Whip is his son and joins Michael’s plan. Whip is fatally shot, and T-Bag kills his attacker as Michael exposes Jacob’s operation. Sara and Michael rescue their boy and turn evidence against Jacob over to the authorities. Jacob is imprisoned, and Michael regains his freedom with his family; the season ends with him, Sara and their son together, while T-Bag returns to prison.
