Eko sees a figure resembling Yemi asking him to confess before his shelter catches fire. Rescued by Charlie and Hurley, he disappears into the jungle. Locke organizes a trip to the Pearl station, hoping its equipment can help contact the Others. The party finds Eko near the aircraft, but Yemi's body is missing. Eko stays outside while the others explore the station and discover a live monitor showing a man with an eyepatch, who switches off his camera.
Flashbacks show Eko assuming Yemi's priestly role in Nigeria and confronting militia demands for the village's vaccine supplies. Eko plans to sell the vaccine himself, bringing the men back to threaten him. He kills them inside the church, after which the villagers board it up as desecrated. Amina challenges his conduct and tells him he owes his brother a church. On the island, Eko refuses the apparent Yemi's demand for repentance, insisting that he acted to survive.
The figure denies being Eko's brother and retreats. The smoke monster then attacks Eko, throwing him against trees and leaving him mortally wounded. Locke reaches him before he dies and later reports his final warning that they are next. Separately, Ben acknowledges needing Jack's surgical help. Juliet plays a video of herself displaying written messages while she speaks aloud about the operation. The cards secretly urge Jack to make Ben's death appear accidental and promise him protection.
