Jack returns to the beach with Kate, Sayid and Juliet, but his assurances do not persuade everyone to trust their former captor. Claire becomes seriously ill, and Juliet claims to know the cause because it relates to treatment designed during Claire's pregnancy. She retrieves medicine from a concealed drop point. Sayid and Sawyer follow and demand answers, but Juliet challenges them over their own violent histories and returns with the supplies. After the injection, Claire recovers, helping Juliet gain acceptance. Jack brings her materials for a shelter and explains that he trusts her because he saw how badly she wanted to leave the island with him.
Flashbacks follow Juliet's arrival after she says goodbye to her pregnant sister Rachel. Richard persuades her to drink a sedative, and she wakes aboard the submarine before meeting Ben. Her work fails to prevent a pregnant patient's death, and Ben refuses her request to return home. He claims Rachel's cancer has returned and promises that Jacob can cure it if Juliet stays. Years later, he shows her a live video of Rachel with her healthy child. The final recollection reveals that Juliet's abandonment was staged. Ben instructed her to handcuff herself to Kate and use Claire's deliberately activated illness to earn the survivors' confidence. Juliet assembles her shelter while keeping that agreement concealed from the camp.
